Disability Support Coordinator

The RoleThis is an exciting opportunity for a highly motivated and organised individual to take on a leadership role within our team. As a Case Coordinator, you will be responsible for supporting the team in achieving set targets and meeting organisational and department goals.
- Oversee and implement individual plans with a person-centred approach
- Assist in the intake and induction of new participants
- Manage and monitor staff, including behaviour plans and incident reporting
- Build relationships with stakeholders for positive participant outcomes
- Attend relevant NDIS and specialist appointments
- Review participant profiles and introduce new activities
- Facilitate communication and advocate for participants' needs
- Supervise and manage Team Leaders
- Provide training and mentoring to Team Leaders
- Maintain documentation, reporting, and safeguarding requirements
We offer a competitive salary package, including a SCHADS Award Level 4 - Pay Point 1 salary, plus benefits such as flexible working hours and opportunities for professional development.
RequirementsTo be successful in this role, you will require:
- A minimum of 1 year's experience in case coordination or a related field
- Relevant qualifications, including a Certificate IV in Individual Support or equivalent
- Current NDIS Worker Screening and NDIS Worker Orientation Module
- First aid and CPR certification
- A current national police check (or willingness to obtain)
- An open driver's licence
- Evidence of right to work in Australia
The salary for this role is $65,000 per annum, plus superannuation and other benefits.
