The Role

This is an exciting opportunity for a highly motivated and organised individual to take on a leadership role within our team. As a Case Coordinator, you will be responsible for supporting the team in achieving set targets and meeting organisational and department goals.

  • Oversee and implement individual plans with a person-centred approach
  • Assist in the intake and induction of new participants
  • Manage and monitor staff, including behaviour plans and incident reporting
  • Build relationships with stakeholders for positive participant outcomes
  • Attend relevant NDIS and specialist appointments
  • Review participant profiles and introduce new activities
  • Facilitate communication and advocate for participants' needs
  • Supervise and manage Team Leaders
  • Provide training and mentoring to Team Leaders
  • Maintain documentation, reporting, and safeguarding requirements

Requirements

To be successful in this role, you will require:

  • A minimum of 1 year's experience in case coordination or a related field
  • Relevant qualifications, including a Certificate IV in Individual Support or equivalent
  • Current NDIS Worker Screening and NDIS Worker Orientation Module
  • First aid and CPR certification
  • A current national police check (or willingness to obtain)
  • An open driver's licence
  • Evidence of right to work in Australia
Salary

The salary for this role is $65,000 per annum, plus superannuation and other benefits.
