Remote Area Chronic Disease Care Specialist

This is an exceptional opportunity to work as a Nurse/Midwife Clinical Specialist, delivering expert care and education to improve chronic condition management across remote NT communities.
As a specialist in preventable chronic conditions, you will provide outreach care to remote clients and support PPHC staff with the focus on improving outcomes in early detection and management of these conditions in a culturally sensitive manner.
About the Role- You must be currently registered with the Nursing and Midwifery Board of Australia or eligible for same.
- A high-level clinical knowledge and skills in Preventable Chronic Conditions or related field, supported by strong decision-making and problem-solving skills are essential.
- The successful candidate will demonstrate a commitment to a learning culture through clinical guidance and mentoring.
- You will travel to remote communities by 4WD or light aircraft as required.
- A current NT driver's licence (or eligible to obtain) and confidence driving a manual vehicle in regional and remote settings are also required.
