Job Title: Regional Care Coordinator

The Care Coordinator role at our organization presents a rewarding opportunity to make a positive impact on the lives of Aboriginal people with chronic conditions. We are seeking an experienced and dedicated professional to join our team as a Regional Care Coordinator.

This position involves collaborating with clients, general practitioners, clinic staff, and other health service providers to provide care services that optimize health outcomes for Aboriginal individuals. The successful candidate will possess excellent communication and interpersonal skills, as well as a strong understanding of primary healthcare and sound clinical knowledge of chronic disease management.

To be considered for this position, you will require:

  • Qualifications: Registered or eligible to register with AHPRA as a registered nurse or Aboriginal Health Practitioner with at least 2 years clinical experience in chronic disease management and/or primary health care
  • Skills: Excellent understanding of primary health care and sound clinical knowledge of chronic disease management
  • Aboriginal Health Understanding: Understanding and commitment to Aboriginal health, and the philosophy and practice of Aboriginal Community Control and knowledge of current issues affecting the lives of Aboriginal people
