Regional Healthcare Facilitator

This position plays a vital role in our organization, requiring collaboration with clients, healthcare professionals, and other service providers to deliver high-quality care services to Aboriginal people with chronic conditions.
Success in this role demands adaptability, as you will need to work in remote areas and support clients who require travel to Alice Springs or interstate for specialist appointments.
- Key Responsibilities:
- Collaborate with clients, healthcare professionals, and other service providers to provide care services for Aboriginal people with chronic conditions.
- Support clients requiring travel to Alice Springs or interstate for specialist appointments.
- Work in remote areas to provide care services to Aboriginal people with chronic conditions.
Requirements:
- Registration with AHPRA as a registered nurse or eligibility to register as an Aboriginal Health Practitioner with at least 2 years of clinical experience in chronic disease management and/or primary health care.
- Strong understanding of primary health care and sound clinical knowledge of chronic disease management.
- Understanding and commitment to Aboriginal health, and the philosophy and practice of Aboriginal Community Control, as well as knowledge of current issues affecting the lives of Aboriginal people.
