Job Title: Harm Reduction Outreach Specialist
Program Overview:SAFER is a community-based program that increases health equity by providing safer alternatives to the toxic drug supply.
Responsibilities:- Provide individualized care and support for participants with complex health needs.
- Work collaboratively with the team to provide harm reduction services through risk reduction strategies.
- Develop and maintain ongoing monitoring and support plans, ensuring seamless collaboration with community partners.
- Registered Nurse, Registered Psychiatric Nurse, or Licensed Practical Nurse registered with the relevant regulator.
- Minimum 1-5 years of full scope, outreach nursing experience working with individuals with high levels of poly-substance use and mental health challenges.
- Commitment to social justice principles, including harm reduction, anti-racism, decolonization, and combating stigma related to drug use.
- Establishing respectful rapport with participants.
- Observing and recognizing subtle changes in participants.
- Working independently and as part of a multidisciplinary team.
- Excellent communication, conflict resolution, and crisis management skills.
