Job Title: Harm Reduction Outreach Specialist

Program Overview:

SAFER is a community-based program that increases health equity by providing safer alternatives to the toxic drug supply.

Responsibilities:
  • Provide individualized care and support for participants with complex health needs.
  • Work collaboratively with the team to provide harm reduction services through risk reduction strategies.
  • Develop and maintain ongoing monitoring and support plans, ensuring seamless collaboration with community partners.
Qualifications:
  • Registered Nurse, Registered Psychiatric Nurse, or Licensed Practical Nurse registered with the relevant regulator.
  • Minimum 1-5 years of full scope, outreach nursing experience working with individuals with high levels of poly-substance use and mental health challenges.
  • Commitment to social justice principles, including harm reduction, anti-racism, decolonization, and combating stigma related to drug use.
Skills and Abilities:
  • Establishing respectful rapport with participants.
  • Observing and recognizing subtle changes in participants.
  • Working independently and as part of a multidisciplinary team.
  • Excellent communication, conflict resolution, and crisis management skills.
