
Caregiver - Community Outreach

As a Home and Family Support Specialist, you will play a critical role in creating a nurturing atmosphere of home and family. You will work as part of a team to identify clients' problems, needs, and risks, and develop individualized support plans to address these issues.
The ideal candidate will have a diploma in a related human/social service field and a minimum of one year recent experience in a similar role. They will possess strong problem-solving, decision-making, communication, and collaboration skills, as well as knowledge of non-violent crisis intervention techniques and harm reduction approaches.
You will be responsible for providing direct support to residents, including conducting individual counselling sessions, developing rapport and trusting relationships, and assisting with daily living tasks such as laundry and meal distribution. You will also participate in cross-agency working groups and collaborate with partner agencies and other professionals/support teams as needed.
The Home and Family Support Specialist will maintain accurate records and statistics, report on client progress, and provide recommendations for program improvements. They will also uphold all policies and procedures, including those related to confidentiality, boundaries, and de-escalation techniques.
