Job Summary:

As an Aged Care Specialist, you will be responsible for undertaking comprehensive assessments to formulate and implement appropriate care plans for older individuals. You will work collaboratively with multidisciplinary teams and independently as a clinician, utilizing your knowledge of My Aged Care, related programs, and services to support routine and complex aged care presentations.

Key Responsibilities:
  • Conduct thorough assessments to identify the needs of older individuals and develop tailored care plans
  • Collaborate with healthcare professionals and other stakeholders to ensure seamless care delivery
  • Utilize electronic patient record systems and Microsoft Office Programs to manage client information and communicate effectively
Requirements:
  • Relevant tertiary qualifications in a clinical discipline or a related field
  • Full registration with the Australian Health Practitioner Regulation Agency (AHPRA) or eligibility for membership of relevant professional bodies
  • Strong written and verbal communication skills, with proficiency in electronic patient record systems and Microsoft Office Programs
