Job Title: Senior Aged Care Specialist

We are seeking a highly skilled and experienced Senior Aged Care Specialist to join our multidisciplinary team. The successful candidate will play a crucial role in providing expert leadership and clinical expertise to our Aged Care Rapid Response Team.

The ideal candidate will have a strong background in aged care, with a minimum of 5 years of experience in a clinical leadership role. They will possess excellent communication and interpersonal skills, with the ability to work effectively within a multidisciplinary framework.

The Senior Aged Care Specialist will be responsible for coordinating comprehensive hospital avoidance strategies, facilitating fast-track diagnostic, assessment, and treatment, and providing effective short-term case management for older people.

Key Responsibilities:

  • Provide clinical supervision to nursing staff and act as a role model, providing nursing care on a regular basis.
  • Formulate and implement policy, guide practice development, and engage in education and research activities.
  • Develop and maintain relationships with stakeholders, including Residential Aged Care Facilities (RACFs) staff, healthcare providers, and community organizations.
  • Monitor and evaluate the effectiveness of hospital avoidance strategies and make recommendations for improvement.

Required Skills and Qualifications:

  • Registered with Authority to Practice with Australian Health Practitioner Regulation Agency.
  • Minimum 5 years full-time post-graduate experience with 3 years full-time equivalent in the specialty field.
  • Approved post-graduate qualification or evidence of working towards Masters level qualification.
  • Clinical leadership skills and ability to work collaboratively within a multidisciplinary framework.
  • Effective communication and interpersonal skills across various mediums.
