Job Title: Aged Care Specialist

As an Aged Care Specialist, you will be working with a dynamic team across various sites in the Northern Sydney Local Health District, providing aged care assessment services.

You will be part of an experienced and friendly team of Occupational Therapists (OTs), Physiotherapists (PTs), Social Workers (SWs) and Registered Nurses (RNs).

  • Qualifications: Relevant tertiary qualifications as per clinical discipline, with full registration or eligibility for membership of the relevant Australian peak professional body.
  • Communication Skills: Proven high standard of written and verbal communication skills, with ability to use Microsoft Office Programs, electronic patient record systems and e-Learning sites.
  • Clinical Experience: Demonstrated discipline in and clinical experience working in relevant aged care settings, with proven ability to undertake comprehensive assessments and formulate care plans.
  • Aged Care Knowledge: Detailed knowledge of My Aged Care, related aged care programs and services, with ability to assess and support routine and complex aged care presentations.
