  • This Senior Care Professional will conduct initial client intake and screening assessments, developing and reviewing individualized care plans with a client-centered approach, providing quality care based on clients' needs.

The ideal candidate will have experience conducting thorough initial screening assessments, managing client information in CRM & CMS, and engaging in ongoing care discussions and planning.

Key Responsibilities:
  1. Conduct comprehensive assessments to develop personalized care plans that cater to each client's unique needs.
  2. Manage client information effectively using CRM and CMS systems.
  3. Engage in ongoing care discussions and planning with the care team to ensure optimal support for clients.

Additional responsibilities include maintaining accurate records, interacting one-on-one with clients, and providing support with daily activities.

Required Skills and Qualifications:

To succeed in this role, you will need:

  • A Certificate III in Aged Care or Individual Support, or an equivalent qualification.
  • A current Australian driver's license (Green Provisional minimum).
  • Experience with technology, including smartphones, tablets, laptops, and other devices.
