Advanced Palliative Care Clinician

This challenging role offers an opportunity to work as a highly skilled and autonomous healthcare professional in our palliative care service. As a key member of the multidisciplinary team, you will be responsible for providing expert clinical assessments and implementing advanced care plans for complex and evolving patient needs.

The ideal candidate will possess a registered nursing qualification, an endorsement as a nurse practitioner, and advanced skills in diagnostics, assessment, referral, and treatment planning. A strong background in palliative care models and interdisciplinary teamwork is also essential. Leadership capabilities with a focus on education, mentorship, and service development are required.

  • Conduct Comprehensive Assessments: Conduct thorough assessments of patients' physical, emotional, and social needs to develop and implement individualised care plans.
  • Collaborate with Multidisciplinary Team: Work closely with doctors, allied health professionals, and other healthcare teams to ensure seamless care transitions and optimal patient outcomes.
  • Develop Advanced Care Plans: Create and implement advanced care plans that cater to the unique needs of patients, focusing on pain management, symptom control, and quality of life.
  • Provide Education and Mentorship: Educate and mentor nursing and medical staff on best practices in palliative care, promoting a culture of excellence and continuous learning.
  • Participate in Quality Improvement Initiatives: Contribute to the development and implementation of quality improvement projects aimed at enhancing patient safety, satisfaction, and outcomes.
  • Support Research and Development: Participate in research studies and contribute to the development of new protocols and guidelines that advance palliative care practices.
  • Engage with the Community: Foster relationships with local communities, hospitals, and healthcare organisations to promote services and support the integration of palliative care into primary healthcare settings.
