Deliver transformative End of Life care initiatives across a dynamic healthcare district.
This role provides strategic leadership in designing, integrating, and implementing high-quality End of Life care models. You will drive alignment between subspecialties, care settings, and partners to ensure person-centred care that meets the NSW Health End of Life and Palliative Care Framework and Clinical Principles for End of Life and Palliative Care Guideline.
You will work collaboratively with inpatient, community, and specialist palliative care services to build partnerships with internal and external stakeholders. Through effective change management and evidence-based innovation, you will foster integrated, sustainable models of care that address local needs while meeting statewide strategic priorities.
