
Mental Health Care Specialist

We are seeking an experienced and compassionate Mental Health Care Specialist to join our team. As a key member of our multidisciplinary team, you will be responsible for providing case management and assessment services to older adults with mental health needs.
Key Responsibilities:- Case Management and Assessment: Develop and implement individualized care plans to address the unique needs of older adults with mental health concerns.
- Collaboration and Communication: Work effectively with multidisciplinary teams, including healthcare professionals, families, and community support services to ensure seamless care coordination.
- Community-Focused Care: Provide person-centred care that takes into account the social determinants of health and promotes community engagement and participation.
- Professional Qualifications: Current registration with AHPRA as a Registered Nurse.
- Vaccination Requirements: Compliance with applicable vaccination requirements.
- Driver's Licence: Valid NSW Driver's Licence.
