**Job Title:** Community Support Specialist

We are seeking a compassionate and motivated individual to work with our clients, assessing their needs and delivering tailored support services.

The successful candidate will be responsible for:

  • Assessing client needs and developing personalized plans
  • Conducting interviews to determine the nature and extent of difficulties faced by clients
  • Monitoring and reporting on client progress and adapting support plans as necessary
  • Referring clients to external agencies and community resources for further assistance

**Requirements:**

  • Certificate in Community Services or related field (preferred but not required)
  • Proven experience in client assessment, case management, and community engagement
  • Excellent interpersonal and communication skills
