The Role:
We are seeking a passionate Grade 2 Occupational Therapist to join our Community Therapy team at Northern Health. As an Occupational Therapist Community Health Specialist, you will provide occupational therapy services to patients/participants within the Commonwealth Home Support Program (CHSP), National Disability Insurance Scheme (NDIS), and Home and Community Care (HACC). These services will support and maintain people living at home whose capacity for independent living is at risk or who are at risk of premature or inappropriate admission to long-term residential care.
The CHSP service specifically provides home and community services for people aged 65 and over. The role also involves providing services to our Progressive Neurological Disease Clinic, including people with Motor Neurone Disease. This position is based primarily at Broadmeadows Hospital.
Key Responsibilities:
- Deliver occupational therapy services to patients/participants within the CHSP, NDIS, and HACC programs
- Work collaboratively with multidisciplinary teams to develop and implement treatment plans
- Conduct assessments and interventions to promote independence and functional ability
- Develop and maintain relationships with clients, families, and carers to ensure person-centered care
Requirements:
- Degree in Occupational Therapy or equivalent qualification
- Registration with the Australian Health Practitioner Regulation Agency (AHPRA)
- Experience working in community health or rehabilitation settings
- Strong communication and interpersonal skills
- A positive attitude to lifelong learning
