Client Services Coordinator
The position is responsible for coordinating client services and ensuring high-quality care. Key responsibilities include:
Determining clients' requirements through intake interviews and reviewing therapist evaluations, treatment objectives, and plans.
Establishing treatment programs by setting schedules and routines, coordinating services, arranging resources, and verifying clients' attendance.
Monitoring cases by observing and evaluating treatments and responses, advocating for needed services and entitlements, obtaining additional resources, and intervening in crises.
Maintaining clients' records by reviewing case notes and logging events and progress.
Communicating clients' progress through weekly interdisciplinary meetings and evaluations, disseminating results and obstacles to the therapeutic team and family.
Advocating for client needs, choices, and rights, respecting clients' choices of advocate, and including them as needed in providing and identifying service needs.
Preparing clients' discharge by reviewing and amplifying discharge plans, coordinating discharge and post-discharge requirements, orienting and training family members, and providing resources.
Improving treatment results by studying, evaluating, and redesigning processes, implementing changes, rewriting policies and procedures, improving financial status by analyzing results, monitoring variances, identifying trends, and recommending actions to management.
Managing client budgets, complying with, and contributing towards the success of the quality management system, meeting budget by monitoring expenses and implementing cost-saving actions.
Updating job knowledge by participating in educational opportunities, reading professional publications, maintaining personal networks, and participating in professional organizations.
Work Details
Full-time, permanent position.
8-hour shift, Monday to Friday, on call.
$63,000.00 - $70,000.00 per year salary range.
Required Skills and Qualifications
A relevant qualification or willingness to undertake traineeship to obtain a diploma in community services is highly desirable. Experience in case management is preferred but not essential.
