We are seeking an experienced Administrative Care Specialist to join our team at Sydney Local Health District. As a key member of the Out of Hospital Care (OHC) service, you will play a crucial role in supporting hospital discharges and preventing avoidable admissions by providing short and medium-term packages of non-clinical care.

The ideal candidate will have demonstrated experience monitoring clients and linking in with appropriate services. You will be able to follow guidelines relevant to the position as well as demonstrate problem-solving capacity. A current NSW Driver's Licence is also required.

This is a Category A position that requires evidence of vaccination to comply with the NSW Health Occupational Assessment, Screening & Vaccination against Specified Infectious Diseases Policy.

**Salary:** $69,991 - $73,876 per annum + annual leave loading + 11.5% Superannuation
