Community Health Specialist

As a Community Health Professional, you will be working independently with a patient-centered approach to promote health and wellbeing in the community.
The OpportunityWe are seeking an experienced Physiotherapist to join our team, working across Sydney Metro area, particularly the Inner West and Eastern Suburbs, in a permanent full-time capacity.
You will work with a range of clients, in community and occasionally in residential aged care, supporting and guiding them to achieve their goals and ultimately to help them live their best lives.
- Private services: supporting individuals with self-identified needs.
- Rehabilitation services in the home program: focusing on post-orthopaedic surgery
- Hospital-in-the-home program: supporting clients after hospitalization, including general surgeries
- Home Care Package clients: supporting those in our community who wish to remain independent at home
- Residential aged care facilities: developing, implementing, and managing group and 1:1 exercise programs
You are a fully registered Physiotherapist, with ideally 1+ years' experience, ideally within a community or rehabilitation setting and are comfortable working independently.
As a mobile Community Health Professional, ideally you live on or close to the Inner West or Eastern Suburbs and are comfortable travelling to our clients' homes.
