Community Healthcare Specialist

We're seeking an experienced Occupational Therapist to promote health and wellbeing in the community.
Job OverviewThis is a fixed-term contract for parental leave, but may be extended or made permanent.
- Key Responsibilities:
- Conduct home assessments, prescribe equipment and modify homes for clients receiving Home Care Packages.
- Lead rehabilitation programs in client homes, focusing on post-orthopaedic surgery recovery.
- Support clients after hospitalisation through our Hospital in the Home program.
- Requirements:
- Current AHPRA registration as an Occupational Therapist, with experience in community and residential aged care settings.
- Ability to work independently and provide support in various care environments.
- Patient-centred approach and passion for improving health outcomes.
- Comfortable travelling to clients' homes, ideally living in South West Sydney.
