About the Role
This is a full-time permanent opportunity at Uniting.
The Aftercare Coach will walk alongside young people as they take their next steps into independence, helping them set goals, navigate challenges and build skills for a thriving future.
Key Responsibilities
- Support Young People to meet contract, performance and practice requirements, upholding ethical and professional standards.
- Contribute to Practice Improvements, promoting high standards of service delivery in Aftercare.
- Foster Autonomy and Self-Determination by building trusting relationships with young people.
- Provide Advocacy and Support based on trauma-informed care and Advantaged Thinking principles.
- Build Partnerships to facilitate access to entitlements and assistance such as allowances and personal files.
- Review and Implement Plans addressing support gaps and facilitating referrals to relevant services.
- Support Engagement in employment, training and education.
- Facilitate Transitions through post-exit check-ins ensuring ongoing wellbeing and smooth transitions.
- Deliver Intensive Support, including case management, goal planning and service coordination.
- Develop Independent Living Skills and strengthen connections to family, culture and community.
- Deliver Holistic Mentoring and facilitate group sessions on topics including social skills, job readiness, financial literacy and health and wellbeing.
Qualifications and Experience
- A minimum of 3 years' experience working with young people and relevant tertiary qualifications in social science or equivalent.
- A current NSW driver's licence and NSW Working With Children Check (WWCC).
- Exceptional Relationship-Building and Advocacy Skills.
- A 'Can-Do' Attitude, flexibility to adapt to changing needs and priorities.
- Energy to Match the Pace of Young People.
- A commitment to prioritising supervision and case reviews to ensure best practice.
- Passion for Making a Difference through trust, relationship and energy.
- Skill in Goal Setting and Achievement.
- Willingness and Availability to Travel regularly to meet young people where they are located.
- Strong Organisational Skills to meet reporting obligations with accuracy and timeliness.
