About us:

The Youth Initiative (YI) Program is an initiative that aims to prevent homelessness among young people leaving care by building their capacity, skills and capability. The primary purpose of this position is to work alongside young people transitioning out of Out of Home Care (OOHC) services to support and mentor them to develop networks and skills for independence and successful community living.

About the role:

Foundations Care is seeking a Personal Advisor to develop a mentor-like relationship with the YI program's young people. This role will be based at our Tamworth, NSW location and is being offered in a full-time fixed term contract capacity, initially for six (6) months, with the possibility of extension.

The Personal Advisor will be the central point for coordination and mentorship for all service elements accessed by the young person to support them to gradually develop the skills and strategies to manage change and crisis. You will build on the young person's pro-social networks and support the facilitation and implementation of the young person's leaving care plan.

This position will also support the young people to actively achieve plan goals, including supporting the young person to build appropriate study or work skills, providing avenues for employment engagement, develop strategies to address barriers in engagement and celebrating achievements. This involves the understanding of the young person's history, interests, ability and motivation to create and implement an education and/or employment plan with the purpose of achieving engagement in sustained learning and/or earning opportunities.

About you:

  • You will ideally be an enthusiastic, knowledgeable individual with a heart for working alongside young people to assist them to develop the necessary skill to transition into independence.
  • You will show flexibility in your approach with a focus on creating a trusting relationship with young person/s but with a clear goal of achieving planned outcomes aligned with the goals highlighted in the young person's leaving care plan.
  • You will always demonstrate cultural competence in your approach and interactions and will adhere to Foundations Care Ltd core practice and policy frameworks, engaging in self-learning or training opportunities to further your knowledge and skills

  • You will bring your knowledge and networking skills to the role and will demonstrate an ability to focus solely on building resources and strategies to improve positive outcomes for young people in their communities.

  • You will have a minimum of a Certificate IV in related fields of Human and/or Community Services; be enrolled in; or be willing to obtain
