We're seeking a skilled professional to join our team as an Aftercare Coach. In this rewarding role, you will play a vital part in helping young people navigate their journey towards independence.

Key Responsibilities:

  • ">
  • Foster trusting relationships with young people and support them in setting goals and building skills for a thriving future.">
  • Meet contract, performance, and practice requirements, upholding ethical and professional standards.">
  • Contribute to practice improvements in Aftercare, promoting high standards of service delivery.">
  • Provide advocacy, mentoring, and practical support based on trauma-informed care and Advantaged Thinking principles.">
  • Facilitate access to entitlements and assistance such as allowances, Youth Allowance, and personal files.">
  • Review and implement aftercare plans, addressing support gaps and facilitating referrals to relevant services.">
  • Support engagement in employment, training, and education.">
  • Conduct post-exit check-ins to ensure ongoing wellbeing and smooth transitions.">
  • Demonstrate a deep understanding of the difference between a caseworker and the role of a coach – empowering rather than directing.">
  • Show exceptional relationship-building and advocacy skills.">
  • Exhibit a 'can-do' attitude, with flexibility to adapt to changing needs and priorities.">
  • Be available to travel regularly to meet young people where they are regionally located, promote the Specialist Aftercare program, engage in team-building activities and training.">
  • Have strong organisational skills and ability to meet reporting obligations with accuracy and timeliness.">

Requirements:

  • ">
  • A minimum of 3 years' experience working with young people and relevant tertiary qualifications in social science or equivalent.">
  • A current NSW driver's licence and NSW Working With Children Check (WWCC).">
