The role of an Aboriginal Health Worker/Practitioner in a community health setting is pivotal in promoting the health and wellbeing needs of Aboriginal individuals, families, and communities. The position provides direct patient contact and clinical support in areas such as Maternal and Child Health and Chronic Disease Management.
Key Responsibilities:- Establish strong relationships between clients and clinic staff to facilitate effective communication and treatment follow-ups.
- Provide referrals to appropriate services and community health programs, ensuring seamless coordination of care.
- Maintain awareness of local Aboriginal Health issues and advise management and professional staff on strategies for improvement.
- Contribute to preventive and health education activities under the supervision of General Practitioners and nursing staff.
- Promote healthy lifestyles within the community through targeted interventions and outreach programs.
- Collect, analyze, and coordinate health statistics and information processes to inform evidence-based decision-making.
- Develop and implement public health strategies, programs, and services tailored to the specific needs of the Aboriginal community.
- Perform primary care tasks within the scope of your qualifications and expertise.
- Collaborate closely with General Practitioners and relevant clinic staff to develop and deliver comprehensive healthcare plans.
- Build and maintain relationships with local communities to address Indigenous health concerns.
- Assist in the development and delivery of health education programs, workshops, and training sessions.
- Prepare and submit regular reports on time, providing valuable insights into program effectiveness.
- Foster partnerships with allied health professionals and specialists to enhance service delivery.
- Coordinate with other Aboriginal Medical Services (AMS) and hospitals to access client medical records and medication information as necessary.
- Ensure effective communication and rapport-building with external health services.
- Encourage continuous improvement through active participation in quality assurance processes.
- Hold a Certificate III or IV in Aboriginal and/or Torres Strait Islander Primary Health Care, or be working towards achieving this qualification.
- Demonstrate experience in independent patient care and initiative-taking.
- Possess knowledge of specialist medical, allied health, and support services locally.
- Exhibit excellent communication, time management, and organizational skills.
- Understand the complexities impacting Aboriginal people and communities.
- Display knowledge of primary healthcare settings and adaptability in diverse locations.
- Demonstrate computer literacy and high-quality report writing skills.
- Provide a Certificate of Aboriginality.
- Undertake a Working With Children Check.
- Present proof of COVID vaccination.
- Supply Immunisation Record, including Health Worker Vaccinations (DTP Hep B, MMR, Varicella).
- Obtain a National Criminal Record Check.
