Job Summary

We are seeking a skilled Aboriginal Health Worker/Practitioner to join our team in delivering high-quality healthcare services to the Aboriginal community.

As an Aboriginal Health Worker/Practitioner, you will be responsible for providing direct patient contact and clinical support in Maternal and Child Health and Chronic Disease Management.

You will work closely with GP's and relevant clinic staff to assist with all aspects of health care plans and develop and deliver health education programs tailored to the needs of the Aboriginal community.

Responsibilities include:

  • Liaising between clients and clinic staff and assisting with the follow-up of clients' treatments.
  • Providing referral services by referring clients to appropriate services and community health organizations.
  • Monitoring local Aboriginal Health problems and needs, and advising the Clinic Manager and/or professional staff.
  • Participating in or carrying out preventive and health education activities under the supervision of GP's and or Nursing Staff.
  • Promoting healthy lifestyle in the community.
  • Collecting, collating, and coordinating health statistics and information processes for the advancement of Aboriginal health and well-being.
  • Implementing and evaluating public health strategies, programs, and services directed to the needs of the Aboriginal community of the region.
  • Undertaking primary care tasks within the boundaries of your qualifications.
  • Working closely with the GP's and relevant clinic staff to assist with all aspects of the development and delivery of health care plans.
  • Developing and maintaining contact with communities regarding Indigenous health.
  • Assisting in the development and delivery of health education programs.
  • Providing reports as required on time.
  • Liaising with allied health professionals and specialists as required.
  • Liaising with other AMS's and hospitals for client medical and medication records as necessary.
  • Ensuring good rapport and communication with external health services.
  • Actively encouraging and promoting continuous improvement.
  • Liaising with clients for screenings and tests as required.
