We are seeking a skilled and dedicated Registered Nurse to join our team as an NDIS Service Coordinator. This is an exciting opportunity to work with individuals with disabilities and provide high-quality care and support.
The successful candidate will play a crucial part in providing person-centred care and support to our clients, helping them achieve their goals and live more independent lives.
Key responsibilities of the role include managing daily operations across participants, preparing in-home / SIL care assessments and drafting care plans to best meet client requirements, ensuring care aligns with NDIS goals and practice standards and client matching, leading and mentoring support staff to work effectively with complex needs, maintaining rosters and managing workforce performance, upholding compliance with restrictive practices and Quality & Safeguards Commission guidelines, mentoring and supporting staff of the team, and driving performance reviews and staff development plans.
The ideal candidate will have current registration as a Registered Nurse (Division 1) with AHPRA, a strong understanding of the NDIS and disability support services, and at least 2 years' experience as a scheduler, case manager, care coordinator, hospital discharge planner, or NDIS service coordinator.
Requirements for the role include excellent communication and interpersonal skills, the ability to build trust and collaborate effectively with participants, strong problem-solving and decision-making skills, proficiency in managing complex caseloads and maintaining accurate records and documentation, commitment to delivering high-quality, person-centred care and support, and a willingness to travel within the local area to visit participants in their homes or the community.
