NDIS Support Coordinator Role

We are seeking a skilled Support Coordinator to join our team and play a crucial role in helping participants navigate their plans, connect with services, and achieve their goals.

About the Job

This is an exciting opportunity for an experienced Support Coordinator to provide high-quality support to NDIS participants. As a key member of our team, you will be responsible for supporting participants in understanding and implementing their NDIS plans, connecting them with the right services, health professionals, and community resources.

You will work collaboratively with a multidisciplinary team to deliver holistic, person-centred support, ensuring supports align with participant goals and maintaining compliance with NDIS guidelines.

Your Responsibilities
  • Support participants in understanding and implementing their NDIS plans.
  • Connect them with the right services, health professionals, and community resources.
  • Monitor progress, ensuring supports align with participant goals.
  • Maintain compliance with NDIS guidelines while focusing on quality outcomes.
  • Work collaboratively with a multidisciplinary team to deliver holistic, person-centred support.
What We're Looking For
  • Independent Support Coordinators with an existing caseload of people who want to continue receiving your services.
  • Strong knowledge of the NDIS framework.
  • Excellent communication and organisational skills.
  • Ability to work independently while contributing to a team.
  • Relevant qualifications (Social Work, Psychology, Disability, Community Services, etc.) preferred.
