Nephrology Care Coordinator
About the Role:RN care managers are instrumental in revolutionizing the kidney care system by identifying and addressing complex chronic kidney disease (CKD) and end-stage renal disease (ESRD) early on. Their work focuses on comprehensive coordinated care, leveraging patient engagement to craft personalized plans that cater to both clinical and non-clinical needs.
The RN care manager conducts regular assessments virtually to detect potential medical or social issues and changes in the home environment that could pose risks. By following transitional care management (TCM) guidelines, they proactively reach out to patients and providers to evaluate status and address any gaps in care.
This role involves managing diverse patient panels while maintaining adherence to NCQA standards for case management. Key responsibilities include communicating updates to the care team and family members, identifying knowledge gaps, and delivering tailored education and resources.
RN care managers act as liaisons with dialysis facilities to convey care plan updates, ensuring seamless transitions in care and accurate program software records. Utilizing Strive data platforms, they identify trends, anomalies, and areas requiring focus to provide culturally sensitive care that meets the unique needs of patients.
The ideal candidate will possess a comprehensive understanding of case management programs, demonstrated through their ability to deliver high-quality care through clinical judgment, analytical skills, and regular attendance. Participation in quality improvement initiatives and regional interdisciplinary team discussions is also essential in enhancing care coordination and collaboration among clinical team members.
Responsibilities:
- Conducts virtual assessments to identify potential health risks.

Qualifications:
- Bachelor's degree from an accredited school of nursing.
- Current Registered Nurse (RN) license in the state of practice.
- 3+ years combined experience in related field.
- Current BLS or CPR Certification required.
- Reliable transportation for patient visits.
Work Environment:
Includes offices, clinics, and patient homes. Requires in-person patient care, including standing, sitting, walking, pushing, pulling, and lifting.
