We are seeking a compassionate and skilled professional to join our team as a Customer Care Advocate.
As a Customer Care Advocate, you will play a vital role in guiding customers through their care journey, building trusted relationships, and delivering safe, person-centred care.
Key responsibilities include:
- Conducting onboarding assessments and developing holistic health plans
- Coordinating multidisciplinary services and collaborating with clinical teams, families, and external providers
- Supporting customers during care transitions, including hospital discharge or changes in health needs
Requirements:
- Experience in community or aged care, including case management and coordinating multidisciplinary care in the home
- Strong customer focus, with the ability to build trusted relationships and handle sensitive conversations with empathy
- Excellent verbal and written communication skills & technical proficiency with Microsoft Suite and CRM systems
- Knowledge of funding streams, quality standards, audits, and experience supporting care transitions such as discharge planning
