Clinical Nurse Specialist

St Vincent's Health Australia is seeking a Clinical Nurse Specialist to join their team. As a key member of the Palliative Care Service, you will coordinate and lead projects related to patient flow and care coordination.

About the Role:
  • Coordinate, lead and participate in projects related to patient flow and care coordination from one service location to another.
  • Monitor patient wait times and estimated discharge dates through update and management of electronic patient journey board, and identify key blockers (e.g., Waiting for What, W4W).
  • Lead multidisciplinary meetings regarding patient flow and movement; advise and negotiate with referrers; escalate bed blocks as needed.

As a Clinical Nurse Specialist, you will work in a dynamic team to lead the provision of quality care to community clients of the Palliative Care Service. You will provide evidence-based nursing care to people of all ages and cultural groups, including individuals, families and communities.

About You:
  • Currently registered as a Registered Nurse (Division 1) with the Australian Health Practitioner Regulation Agency.
  • Current NSW driver's licence (Class C or above).
  • Relevant post-registration qualifications and at least three years' experience working in palliative care.
  • Demonstrated ability to work within the interdisciplinary team at a senior level with a commitment to ongoing professional development and effective communication and interpersonal skills.
