Job Title: Palliative Care Nurse

  • A Palliative Care Nurse is required to provide high-quality patient education, symptom management, and end-of-life care in the comfort of a client's home or residential accommodation.
  • The ideal candidate will have experience in palliative care nursing and be able to work independently and as part of a team to deliver complex care to clients with terminal diagnoses.

About the Role:

This is an exciting opportunity to join our dynamic clinical team and contribute to providing exceptional care to our clients. As a Palliative Care Nurse, you will be responsible for delivering high-quality patient education, symptom management, and end-of-life care in the comfort of a client's home or residential accommodation.

Key Responsibilities:

  • Support and Education for Clients: Provide support and education to clients with terminal diagnoses and their families and carers.
  • Administration and Preparation of Medications: Administer and prepare s/c medications and CSCI for symptom management.
  • General Nursing Care: Provide general nursing care including simple and complex wound care, IDC/SPC care, PICC dressings.
  • Documentation: Accurately and timely document all client records.
  • Communication: Communicate effectively within the clinical team and community palliative care teams and referrers to ensure care is provided.

Requirements:

  • Credentials: Current AHPRA Nursing registration.
  • Experience: Minimum 2 years' experience in palliative care nursing, with community experience desirable.
  • Qualifications: Post-graduate qualifications (or working towards) in Palliative Care.
  • Clearance: NDIS Clearance (or willingness to obtain one) and Police Clearance within 6 months (or willingness to obtain one).
  • Transportation: Use of own car with KM allowance provided.
  • Licence: Full driver's license and reliable, comprehensively insured car.
