Our organization is seeking a highly skilled and dedicated Nurse Practitioner to join our team in the Palliative Care Service. As a key member of our healthcare team, you will be responsible for providing high-level clinical expertise to support complex and evolving care needs across various settings.

Job Description

We are looking for an experienced and registered Nurse with a strong background in palliative care models and interdisciplinary teamwork. The successful candidate will possess advanced skills in diagnostics, assessment, referral, and treatment planning, as well as leadership capabilities with a focus on education, mentorship, and service development.

As a Nurse Practitioner in our Palliative Care Service, you will work closely with the Community Palliative Care Team (CPCT) to enhance patient outcomes across care environments. This role requires strong communication and collaboration skills, as well as the ability to work effectively in a dynamic and fast-paced environment.

Key Responsibilities
  • Provide expert clinical assessments and implement advanced care plans
  • Lead initiatives in evidence-based practice and clinical service planning
  • Actively contribute to quality improvement and mentoring of staff
  • Collaborate with the Community Palliative Care Team (CPCT) to enhance patient outcomes across care environments
Requirements
  • Registered Nurse with the Nursing & Midwifery Board of Australia including endorsement as a Nurse Practitioner or working towards same as per the NMBA Guidelines on endorsement as a Nurse Practitioner
  • Advanced skills in diagnostics, assessment, referral, and treatment planning
  • Strong background in palliative care models and interdisciplinary teamwork
  • Leadership capabilities with a focus on education, mentorship, and service development
  • NSW Class C Driver's License
