We are seeking a highly skilled and experienced Care Coordinator to join our team. As a Care Coordinator, you will be responsible for managing referrals, onboarding new clients, and adapting care plans to meet changing needs.
Key aspects of this role include:
- Undertaking comprehensive client assessments to develop client profiles and care plans in collaboration with the client/representative and other relevant stakeholders.
- Ensuring care plans are monitored and reviewed on a regular basis to align with the changing needs of clients, including regular client home visits.
- Monitoring the clinical risks and needs of a client, ensuring referral pathways are established for the safety of our clients.
- Collaborating effectively with internal stakeholders to ensure clients' needs are met including our Rostering, Practice and Workforce teams.
- Developing strategies and/or broker services which will address the identified needs of Aboriginal and Torres Strait Islander clients in consultation with families and communities.
- Representing us at external forums and meetings, promoting the organisation within the community.
The successful candidate will have:
- Demonstrated Aged Care or other clinical case management experience.
- Understanding of Aged Care funding and service delivery.
- Confidence in utilising a variety of technology including mobile devices and computers.
- Proficiency with Microsoft Office, Outlook and database systems.
- Ability to effectively build relationships with diverse stakeholders.
- Strong communication skills and confidence in delivering training.
- Ability to multi-task and work autonomously in a fast paced environment.
- A current unrestricted Australian Drivers Licence, reliable vehicle and a willingness to travel.
This is a Full-Time position based in our office, with some travel required to clients' homes across the region. The ideal candidate will possess excellent time management and organisational skills, be able to work effectively as part of a team and maintain high levels of confidentiality.
