Job Overview

We are seeking a skilled professional to provide high-quality care and support to our clients in their homes.

This role involves meeting with clients and their families, assessing their needs, and building strong connections. You will also conduct client care assessments, prepare care plans, and provide nursing care and in-home services as needed.

You will be responsible for handling information requests, feedback, and general enquiries, ensuring all documentation is accurately and comprehensively completed in our databases and software programs.

Additionally, you will interpret health, aged, and disability care policies and regulations for carer and nursing staff, and ensure compliance with local, state, and federal regulatory agencies related to aged and disability care services.

Responsibilities
  • Meet with clients and their families to understand their needs and build strong relationships
  • Conduct thorough client care assessments and develop comprehensive care plans
  • Provide nursing care and in-home services as required, including some medical support
  • Handle client enquiries, provide information, and respond to feedback
  • Maintain accurate and detailed records of client interactions and care provided
  • Stay up-to-date with relevant policies, regulations, and industry developments
Requirements
  • Certificate III Companion Care Level qualification or higher
  • Experience in aged care, disability, or mental health services
  • Understanding of aged care standards and consumer-directed care principles
  • Current First Aid and CPR certification
  • National Police Check
  • WWC (Working with Children) clearance
  • Driver's licence and reliable vehicle with comprehensive insurance
  • High level of computer literacy
  • Strong communication and interpersonal skills
