Job Opportunity

Role Overview: The Aged Care Assessment team conducts comprehensive assessments of clients aged 65+ (50+ for Indigenous clients) to determine their care needs and facilitate access to appropriate services.

As a Senior Clinical Nurse Consultant or Health Practitioner, you will provide leadership and training to clinicians, conducting assessments in hospital or community settings per Commonwealth guidelines and the Aged Care Act 1997. You will focus on social work principles, comprehensive care planning, and ensuring quality evidence-based practice through effective communication and collaboration within the public health system.

Key Responsibilities
