Compassionate Home Care Professional

We are seeking a dedicated and empathetic individual to provide high-quality care and support to our clients in their own homes.
Responsibilities:- Meet with clients and their families to assess their needs and build strong connections
- Conduct client care assessments and prepare care plans to best meet client requirements
- Provide nursing care and in-home services as needed
- Handle information requests, feedback, and general enquiries
- Registered Nurse (RN) with current AHPRA registration
- Experience in community nursing, aged care, disability, or mental health
- First Aid and CPR certification
- National Police Check and Working With Children Check
- Driver's license and reliable vehicle
- Excellent communication and interpersonal skills
- Ability to work autonomously and as part of a team
- Strong problem-solving and time management skills
