This role involves assisting the Administrative Director and Assistant Director with everyday tasks related to the Coordinated Care program. The ideal candidate will be responsible for identifying participants, managing insurance eligibility, scheduling appointments, and performing outreach activities.
Key Responsibilities:
- Identify program participants from internal and external referrals
- Manage insurance eligibility status
- Schedule appointments
- Perform outreach activities
Requirements:
- High school diploma or equivalent
- Strong communication skills with the ability to work independently and collaboratively in a team
- Basic computer knowledge is preferred
