Job Opportunity:

Care Coordinators play a vital role in supporting the health and wellbeing of individuals in rural and remote communities.

As part of our Rural Locum Assistance Program, you will be working with a team of experienced healthcare professionals to deliver essential services to those who need them most.

Key Responsibilities:
  • Providing person-centred care and support to clients with diverse needs, including dementia and high care requirements.
  • Developing, coordinating, and implementing care plans tailored to individual client goals.
  • Supporting clients with mobility, including safe operation of hoists and other manual handling equipment.
  • Collaborating with families, carers, and multidisciplinary teams to ensure holistic care delivery.
  • Maintaining accurate, timely documentation in line with organisational and regulatory standards.
  • Promoting a safe, inclusive, and culturally respectful environment for clients and colleagues.
