We are seeking an experienced Home Healthcare Specialist to join our team. The successful candidate will be responsible for providing premium home care services to clients in their homes.
Responsibilities- Deliver high-quality care that is personalized to each client's needs and preferences.
- Assess clients' health status and develop individualized care plans.
- Provide wound care, diabetes management, and other nursing services as required.
- Collaborate with healthcare teams to ensure seamless care delivery.
- Current registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- Experience in community or subacute care would be highly regarded.
- Ability to work independently and as part of a team.
- Strong communication and interpersonal skills.
- Capacity to maintain accurate records and documentation.
