About this Role
As a skilled healthcare specialist, you will provide quality clinical care to patients in a busy and dynamic environment. Your primary responsibility will be to develop and maintain patient care plans, working closely with the multidisciplinary healthcare team to ensure coordinated patient management.
Key Responsibilities:
• Develop and implement individualized patient care plans that cater to the unique needs of each patient.
• Conduct thorough assessments of patients to identify their medical requirements and coordinate appropriate treatments.
• Collaborate with other healthcare professionals to ensure seamless communication and delivery of high-quality patient care.
• Provide education and support to patients and their families to promote healthy habits and self-management skills.
The Ideal Candidate Will Have:
• The highest levels of personal integrity and ethical behavior.
• Current unconditional and unrestricted registration as a Registered Nurse with the relevant authority.
• A current Certificate in Sexual and Reproductive Health for Nursing or equivalent.
• Minimum of 2 years full-time post-graduate experience within the previous 5 years in a sexual and reproductive health nurse role (or part-time equivalent).
