Lead Care Coordinator
The Lead Care Coordinator is responsible for leading and coordinating all activities related to client care, ensuring the highest quality of service delivery.
Working closely with our team, you will champion the best interests of our clients while driving business goals.
Key Responsibilities:
- Plan, manage, coordinate, and review client goals to ensure effective care outcomes
- Communicate with team members and clients to develop comprehensive care plans
- Document interactions with clients, family, or stakeholders, sharing relevant information with team members
- Meet with clients and families to discuss and review current care plans and goals
Requirements:
- Tertiary qualification in social work, allied health, or a related field (preferred)
- 2-3 years' experience with Home Care Packages and supporting seniors
- Highly organized with strong time management and multitasking skills
- Excellent written and verbal communication abilities
- Valid Australian Driver's Licence required
