Housing and Wellbeing Specialist

We are seeking a skilled professional to work with individuals and families in partnership, empowering them to make effective change and overcome their identified housing and wellbeing needs.

About the Role

This role involves heavy outreach work, engaging individuals and families in crisis accommodation, street outreach and refuges. The successful candidate will translate initial engagement into a strong ongoing partnership.

Key Responsibilities
  • Provide supports and interventions based on the program model of care.
  • Deliver high-quality case management practices and record keeping that adhere to operational requirements.
  • Facilitate client-led assessment and Care Plan to identify client goals, including the entire family where appropriate.
  • Provide targeted interventions and family group meetings to achieve client goals.

The ideal candidate will have excellent communication skills, the ability to work effectively with diverse clients and teams, and a passion for working with Aboriginal and Torres Strait Islander communities.

Requirements
  • Current Cert IV in Social Work, Human Services or related field, with at least 12-months experience in a case management role.
  • OR Current graduate university bachelor's degree in social work, Human Services or related field, with at least a 10-week-full-time placement in social service field.
