Key Responsibilities

As a Case Manager, you will be responsible for overseeing a designated caseload, handling everything from initial referrals to intake and eventual exit.

Actively engage in daily face-to-face client meetings, promptly addressing emergencies in real-time and ensuring positive tenant outcomes through follow-up actions.

  • Develop and implement collaborative individual support plans with residents that include various interventions addressing client needs and goals.
  • Proactively seek feedback from relevant support partners and conduct weekly assessments to devise additional strategies enhancing client support.
  • Closely monitor progress and outcomes of individual case plans, ensuring continuous alignment with client needs and goals.
  • Engage effectively with internal departments, particularly Housing Services, on complex residents and identify collaborative strategies to sustain tenancies.

Deliver proactive, hands-on support to clients, which may include accompanying them to appointments and providing support during these visits.

Actively participate and present at relevant block meetings, resident engagement activities, and new housing sites to raise awareness of support service offerings and the referral process among residents.

Maintain and nurture networks and relationships with external support agencies and community organisations.

Monitor client needs and goals, coordinating timely reviews, surveys, and exit planning as required.
