Professional Case Manager

We are seeking a highly skilled and experienced professional to deliver exceptional case management support services to vulnerable individuals and families.
About the Role:- Conduct thorough assessments of clients' needs and circumstances to develop effective case plans.
- Manage complex situations with professionalism and empathy, ensuring the well-being and safety of all parties involved.
- Identify areas for improvement and implement strategies to address gaps in service delivery.
- Promote collaboration and knowledge-sharing across teams to drive industry development.
- Work closely with Client Services Coordinator and Clinical Practitioner to develop, implement, and review therapeutic case plans.
- Degree qualification in Social Work or related field.
- Proven experience in social service environments, preferably with a focus on homelessness support.
- A valid Driver's Licence.
- A Working with Children's Check.
