We are seeking a skilled professional to fill the role of Case Manager.
The ideal candidate will have experience in client management and be able to connect clients with supports and services. This is a vital role that requires strong communication, advocacy and interpersonal skills, as well as time management and prioritization abilities.
The successful applicant will have a minimum of four years' experience in a case management or client management role, with a diploma or degree in Health or Allied Health field. They must also have a current Australian Driver's Licence and access to a reliable vehicle.
- Key responsibilities include reviewing referral and intake information, undertaking person-centred-care planning, and identifying key referral networks.
- The successful applicant will work closely with clients to set expectations and deliver person-centred-care plans.
