Job Description:

We are seeking an experienced Senior Case Manager to join our team in Yarraville. As a key member of our Kinship Care program, you will work closely with young people, carers, and community partners to ensure the provision of high-quality case management services. This role involves working collaboratively with stakeholders to develop and implement individualised support plans that meet the unique needs of each family.

Key Responsibilities:

  • Provide case management services to kinship carers, focusing on supporting them to consistently meet the developmental needs of children in their care.
  • Collaborate with DFFH Child Protection and other relevant agencies to ensure seamless service delivery and coordination.
  • Develop and maintain strong relationships with carers, young people, and community partners to build trust and confidence in our services.
  • Work effectively in a fast-paced environment, responding to changing circumstances and priorities as needed.
  • Participate in regular case reviews and contribute to the development of new policies and procedures.

Required Skills and Qualifications:

To be successful in this role, you will possess:

  • A passion for working with vulnerable children and young people, particularly those impacted by trauma.
  • Experience in case management, preferably in the context of out-of-home care.
  • Excellent communication, interpersonal, and conflict resolution skills.
  • A good understanding of the legislative, policy, and practice environment surrounding Out of Home Care.
  • Strong administrative skills, including the ability to manage competing priorities and deadlines.
