About the Role

As a highly skilled Mental Health Specialist Case Manager, you will play a pivotal role in providing short to medium-term case management services to consumers presenting with complex mental health needs.

Under the guidance of the Team Manager and Team Consultant Psychiatrist, and in conjunction with team members, you will provide and coordinate targeted therapeutic interventions to support psychosocial recovery, foster individualized recovery, independence, and improved health outcomes for consumers with severe and complex needs.

You will have the opportunity to work closely with multidisciplinary teams using the recovery-focused Strengths Model of Recovery-Oriented Practice. Your expertise and skills will be utilized to develop and implement tailored intervention plans, ensuring that each consumer receives comprehensive support.

Responsibilities
  • Conduct thorough assessments of consumers' needs and develop personalized intervention plans
  • Provide cognitive behavioral therapy, family therapy, peer support, and other targeted interventions as needed
  • Collaborate with healthcare professionals to ensure seamless care coordination and delivery
  • Maintain accurate records and reports to track progress and outcomes
Requirements

To be successful in this role, you will require:

  • A minimum of 2 years post-qualification clinical experience in mental health
  • Registration with the Australian Health Practitioner Regulation Agency (all disciplines except Social Work)
  • A current National criminal history check or willingness to obtain
