At St Vincent's Hospital Melbourne, we are seeking a dedicated and compassionate Mental Health Specialist Case Manager to join our Continuing Care Team.

About the Role

We provide ongoing specialist treatment and support to meet the identified needs of people with a serious mental illness. Our multidisciplinary team uses the recovery-focused Strengths Model of Recovery-Oriented Practice.

As a Mental Health Specialist Case Manager, you will work collaboratively with consumers, families, and healthcare professionals to develop and implement individualized care plans. You will be responsible for providing short- to medium-term case management services, including:

  • Cognitive behavioural therapy
  • Family therapy
  • Peer support
  • Interventions focused on activity/vocation or additional treatment interventions to meet physical health or drug and/or alcohol needs

You will work closely with the Team Manager and Team Consultant Psychiatrist to ensure seamless delivery of services. Your commitment to psychosocial recovery, fostering individualized recovery, independence, and improved health outcomes is essential in supporting our consumers.

Salary

The salary range for this position is $100,932 - $112,091 per annum (RPN Grade 3), $88,030 - $101,19720 per annum (OT/SW Grade 2), or $92,627 - $104,811 per annum (Psychologist Grade 2).

Requirements

To be successful in this role, you will require:

  • A minimum of 2 years post-qualification clinical experience within Mental Health for SW, OT, and RPN
  • Registration with Australian Health Practitioner Regulation Agency (all disciplines except Social Work)
  • Current National criminal history check, or willingness to obtain
