About the Role

The Care Hub is an initiative that brings together various organisations to form a collaborative team of professionals to support Children and Young People in areas such as Mental Health, AOD, Cultural Connection, Family Work, Community Engagement, and Therapeutic Support.

Job Description
  • You will be joining a team of staff helping to support those entering care for the first time.
  • Key responsibilities include assessment, coordinating the care plan, including placement plans, and being the one point of contact for children/young people during their time in the hub.
  • Care coordination involves working with multidisciplinary teams to assess, plan, and provide support to children and young people in the hub.
Required Skills and Qualifications
  • Experience managing risk and wellbeing, working within the Best Interest Framework.
  • A solid understanding of how trauma can affect the development of young people.
  • An understanding of the Child Protection and Welfare system in Victoria.
  • Coordination and facilitation skills for care team meetings and professional meetings with young people, families, and other agencies.
