
Life Coach for Individuals with Intellectual and Developmental Disabilities

Job Description: We are seeking a skilled Life Coach to join our team. As a Life Coach, you will work closely with individuals with intellectual and developmental disabilities to identify and achieve their long-term and short-term goals. This role requires strong communication and interpersonal skills, as well as the ability to develop and implement personalized life plans.
Responsibilities:
- Advocate for and with members to ensure informed decision making and consent
- Schedule, lead, and collaborate with members and interdisciplinary teams to conduct meetings and assessments
- Implement, update, and monitor life plans, facilitating individualized reviews and approval processes
- Ensure integration of all needed and preferred supports and services
- Maintain ongoing contact with critical people in a member's life
- Submit timely documentation, including life plans and progress notes
- Assist members with benefits such as Social Security, Medicaid, and Medicare coverage
- Help members resolve problems in living, including housing, utilities, and safety
- Report abuse or neglect immediately when observed or reported
Required Skills and Qualifications:
- Bachelor's degree with 2+ years of relevant experience, or equivalent combination
- Strong communication, interpersonal, and organizational skills
- Excellent writing and verbal communication skills
