We are seeking a Wellbeing Practitioner to empower veterans and their families to achieve positive outcomes.

About the Role

This role focuses on building therapeutic relationships with veterans and their families by partnering them with local agencies, health practitioners, Government departments, and other ex-service organisations. By responding to and working with our ex-service community, you will be empowering veterans to achieve positive outcomes.

  • Work with veterans and their families within homelessness and financial hardship programs.
  • Assess clients and prepare case management plans that support an outcomes-based approach to veteran wellbeing.
  • Conduct home and hospital visits to veterans to ensure they receive emotional support, counselling or other assistance as required.

The ideal candidate will have a Degree qualification in Humanities/Arts (Social Welfare) or Health Sciences or Social Science, along with 3-5 years' experience in a relevant position. They will need extensive knowledge and experience in Strength-based Case Management, a proven aptitude for completing ATDP or MILADW001 in Welfare, and demonstrated proficiency in planning and managing client workloads across sensitive environments and situations.

Key Requirements
  • A completed Degree qualification in Humanities/Arts (Social Welfare) or Health Sciences or Social Science.
  • A minimum of 3-5 years' experience in a relevant position.
  • Extensive knowledge and experience in Strength-based Case Management.
  • An aptitude for and interest in completing ATDP or MILADW001 in Welfare.
  • Demonstrated proficiency in planning and managing client workloads across sensitive environments and situations.
